WIDE TRACHEO-ESOPHAGEAL FISTULA DUE TO PENETRATION OF AN ESOPHAGEAL STENT

Abstract
Although dysphagia due to esophageal strictures is common treated with esophageal dilations, esophageal stent placement may be required in resistant cases. While endoscopic placement of these stents is not complicated, stents themselves may cause serious complications in adjacent organs. We present a 58-years-old female patient, who presents with dyspnea, cough. Medical history consisted of total laryngectomy operation 20 years ago, and esophageal stent placement 5 years ago. Flexible endoscopic examination through the tracheal stoma revealed a 5 cm wide erosion at the posterior wall of trachea, and the esophageal stent penetrating into the trachea. Due to high risk of surgery, the patient received symptomatic medical treatment. It should be kept in mind that esophageal stents may erode adjacent structures and cause serious complications.

Özet
Özofageal darlıklara bağlı disfajiler sıklıkla dilatasyonlarla tedavi edilse de, dirençli vakalarda özofageal stent yerleştirilmesi gerekebilmektedir. Bu stentlerin endoskobik olarak yerleştirilmesi basit olsa da, stentin kendisi çevre dokularda ciddi komplikasyonlara neden olabilmektedir. Bu yazımızda, solunum sıkıntısı ve öksürük şikayetiyle başvuran, 58 yaşında bir kadın hastayı sunduk. Tıbbi geçmişinde 20 yıl önce geçirilmiş total larenjektomi operasyonu ve 5 yıl önce yerleştirilmiş özofageal stent mevcuttu. Trakeal stomadan girilerek yapılan fleksibl endoskobik muayenede trakea arka duvarında 5 cm genişliğinde erozyon, ve özofagus stentin trakea içine penetre olduğu gözlendi. Cerrahi müdahalenin yüksek riskli olması nedeniyle hastaya semptomatik medikal tedavi uygulandı. Özofageal stentlerin çevre dokuları erozyona uğratabildiği ve ciddi komplikasyonlara sebep olabildiği akılda tutulmalıdır.
