A HIDDEN PRIMARY FOCUS CAUSING BILATERAL NECK METASTASIS: POSTERIOR SURFACE OF UVULA

Abstract
In patients with neck metastasis, it is essential to determine the primary for treatment and survival. Diagnostic procedures consist of a careful clinical evaluation, physical examination including fiberoptic endoscopy, and radiological imaging. The primary tumor is usually found in the upper aerodigestive tract. A 63 year old male was referred to our clinic complaining of bilateral painless neck mass. The primary focus was found at the posterior surface of the uvula after a careful fiberoptic endoscopic examination. THe diameter of the tumor was less than 1 cm without invading the midline of the soft palate. Due to the location of the uvula in the midline of the soft palate, there is a higher risk of bilateral lymph node metastasis. Although cancers of the uvula are rare, they are usually on the anterior surface. We report a rare case of carcinoma arising on the posterior surface of the uvula.

Boyun metastazı olan hastalarda tedavi ve sağ kalım için primerin belirlenmesi esastır. Teşhis prosedürleri, dikkatli bir klinik değerlendirme, fiberoptik endoskopiyi içeren fizik muayene ve radyolojik görüntülemelerden oluşur. Primer tümör genellikle üst aerodijestif traktta bulunur. 63 yaşında erkek hasta boyunda iki taraflı ağrısız kitle şikayeti ile kliniğimize refere edildi. Dikkatli bir fiberoptik endoskopik incelemeden sonra uvulanın arka yüzeyinde primer odak bulundu. Tümörün çapı 1 cm’den küçük idi ve yumuşak damak orta hattını invaze etmiyordu. Uvula yumuşak damak orta hattında yer alması nedeniyle bilateral lenf nodu metastazı riski daha yüksektir. Uvula kanserleri nadir olmakla birlikte genellikle ön yüzeyde yerleşirler. Bu yazıda uvula arka yüzeyinden kaynaklanan nadir bir karsinom vakası sunuldu.
