AbstractBackgorund: Despite recent improvements in both diagnosis and treatment, rhino-orbital-cerebral mucormycosis (ROCM) is a life-threatening infection with a high mortality rate. Early diagnosis and treatment are crucial in terms of mortality and morbidity.Case: A 72-year-old female patient was hospitalised with the diagnosis of type 2 diabetes mellitus (DM), allergic asthma, and ROCM and discharged after iv liposomal amphotericin B treatment and surgical debridement. The patient, who was discharged after surgical and medical intervention, died due to the development of primary adrenal insufficiency. Conclusion: Lipid formulations of Amphotericin B and aggressive debridement form the basis of treatment. The most critical decision is whether the orbit should be exenterated or not. The development of side effects related to antifungal treatment has an impact on the mortality and morbidity of patients. |

ÖzetGiriş: Son yıllarda tanı ve tedavideki gelişmelere rağmen, rino-orbital-serebral mukormikozis (ROCM) yüksek mortalite oranına sahip, yaşamı tehdit eden bir enfeksiyondur. Erken tanı ve tedavi, mortalite ve morbidite açısından büyük önem taşımaktadır.Olgu: 72 yaşında kadın hasta, tip 2 diabetes mellitus (DM), alerjik astım ve ROCM tanısıyla hastaneye yatırılmış ve intravenöz liposomal amfoterisin B tedavisi ile cerrahi debridman sonrası taburcu edilmiştir. Cerrahi ve medikal müdahalelerin ardından taburcu edilen hasta, primer adrenal yetmezlik gelişmesi nedeniyle yaşamını yitirmiştir. Sonuç: Amfoterisin B’nin lipit formülasyonları ve agresif debridman tedavinin temelini oluşturmaktadır. En kritik karar, orbitanın eksenterasyonuna gerek olup olmadığıdır. Antifungal tedaviye bağlı gelişen yan etkiler, hastaların mortalite ve morbiditesi üzerinde önemli bir etkiye sahiptir. |
